Brain Scans Reveal Hidden Risks in Bleeding Strokes

A team of neurologists in London set out to understand why some brain injuries lead to worse outcomes after bleeding strokes. They tracked patients who had suffered acute intracerebral hemorrhages within a specialized stroke program. Using cutting-edge MRI technology, they searched for specific signal patterns known as DWI-positive lesions. This research aims to connect early brain changes with long-term disability. The goal was clear: find out how these invisible markers affect recovery chances.

When the study finished, 74 out of 342 patients—about 22 percent—had these distinctive brain markers. The majority of them lived in the frontal lobes, which handle many important daily functions like speaking and moving. A clear split emerged when comparing patients with cerebral amyloid angiopathy to those without. The former group showed mostly surface-level lesions near the brain's outer layer, while the latter group had deeper ones buried inside.

Those with DWI-positive lesions faced significantly harder recovery journeys. Their brain white matter damage was far more severe on average than their peers. They also carried higher numbers of tiny blood clots throughout the brain tissue. Additionally, their initial stroke volumes were considerably larger, measuring up to 14 milliliters versus 7 milliliters in the other group. Together, these factors explained why some patients scored much lower on the disability scale six months later.
