Brazilian Cardiology's New Protocol: A Step in the Right Direction?

Cardiac rehabilitation programs rely heavily on risk stratification protocols to ensure patients receive the right care. However, these protocols often have different classifications, making it difficult to apply them consistently. In 2020, the Brazilian Society of Cardiology updated its protocol, but its agreement with other established protocols and how well different evaluators agree on its application were unclear.

To address this, researchers conducted a study to evaluate the agreement between the new Brazilian Society of Cardiology protocol and other protocols used in clinical practice. They also looked at how well different evaluators agreed on its application.

The study involved analyzing the medical records of 51 patients in a cardiac rehabilitation program at a public university in Brazil. Two cardiovascular physical therapists independently evaluated these records using the new Brazilian Society of Cardiology protocol, as well as four other established protocols. The researchers used a statistical measure called the Kappa coefficient to assess agreement between the protocols and between the evaluators.

The results showed that the new Brazilian Society of Cardiology protocol agreed moderately with two other protocols and strongly with a third. However, it showed weak agreement with a fourth protocol. The evaluators also showed excellent agreement with each other when applying the protocol.

These findings provide some evidence that the new Brazilian Society of Cardiology protocol is a reliable tool for cardiac rehabilitation programs. However, more research is needed to confirm its effectiveness in predicting exercise-related events.
