Renewed commitments to primary health care and integrated cardiovascular-kidney-metabolic care are creating momentum for change in the Americas. However, persistent gaps between evidence, policy, and implementation are holding back progress. These gaps mainly stem from limitations in organizing health service delivery.

Health systems in the Americas are facing a challenge. They need to find a way to scale up integrated cardiovascular-kidney-metabolic care through primary health care. This requires a new approach, one that aligns actors, strategies, and investments around scalable delivery of existing solutions.

The HEARTS 2.0 program is a practical pathway to accelerate delivery of integrated CKM care. It draws on experience in the Americas and offers a six-lever implementation framework to help close the gaps between evidence, policy, and implementation.
