Fuel on the High Seas: A Hidden Health Risk for Migrants

Every year, thousands of people cross the Mediterranean on overcrowded boats. Sometimes, fuel leaks into the bottom of the vessel, mixing with seawater. Those on board can end up with skin irritation and other health problems after hours of contact. Since last spring, doctors in Lampedusa have seen a rise in migrants arriving with dangerously high levels of methemoglobin in their blood. The cases appeared without a clear cause, which worried the medical teams treating them.

To find out what was happening, a group of clinicians and toxicologists joined forces. They took blood samples from some of the affected migrants and also tested fuel taken from one of the boats. The results showed that 37 out of 39 patients had N-methylaniline in their system. This chemical is a common gasoline additive. The fuel sample itself contained about 1.1 percent of the substance. The pattern suggested that the additive was getting into the body through the skin, especially after long exposure to tainted water.

The standard treatment, methylene blue, was given to 90 percent of the patients, often more than once. Despite this, 38 of the 82 patients developed hemolytic anemia, a condition where red blood cells break down prematurely. Many of those needed blood transfusions to stay stable. Tragically, two patients did not make it. The investigation makes it clear that the gasoline additive is the likely driver of these severe episodes. The study sends an important message for health workers in similar situations. When migrants show up after long sea trips with possible fuel contact, methemoglobinemia should be on the radar. Quick access to methylene blue and the ability to test for the right chemicals can make a real difference. The research highlights that hidden dangers can travel across oceans, and that fast, smart diagnosis is crucial.
