State by State: The ECMO Payment Gap

ECMO stands for extracorporeal membrane oxygenation. It is a machine that takes over heart and lung work for very sick patients. In the U.S., hospitals receive Medicare money for each ECMO discharge. The amount changes based on where the hospital sits. The treatment saves lives, but the money behind it is not equal across the map.

New Hampshire leads with the highest average payment, around $21,000 per patient. Oklahoma has the lowest, at about $5,500. That difference is more than three times the size. Patients in different states get very different financial support for the same care. The gap between the highest and lowest states is huge, and it affects who can afford the care they need.

Many people think state wealth drives the payment size. However, the study found GDP only has a weak link to reimbursement, with a correlation of 0.18. This means richer states do not always pay more. Other factors, not just economics, shape the money flow. The low number tells us the link is not strong enough to predict who gets paid what.

The research shows payments across states vary more than threefold. That huge spread raises questions about how health care dollars are distributed. Looking at the data, it seems geography and policy matter as much as patient need. It also makes us wonder how patients and families experience these differences in real life.
