Young Athletes’ Wrist Bones: When Extra Ulna Leads to Pain
Young athletes often develop a condition where the ulna bone appears longer than the radius on an X‑ray. This difference is called ulnar variance. When the ulna sticks out more, it can create extra pressure on the wrist area. Doctors notice this when they look at the growth plates in the radius, especially in kids whose bones are still growing.
To figure out how much variance exists, clinicians use a standard method that compares the heights of the two bones on a front‑view X‑ray. This technique is often referred to as Hafner’s approach, but the idea is simple: measure which bone extends higher. Studies show that active youngsters typically have a larger ulna side than people who do not play sports.
The exact frequency of this extra ulna side varies a lot across research, ranging from none of the participants to about half. Interestingly, the amount of training does not seem to line up with how much variance appears, and many athletes do not feel pain even when the measurement is high. Some scientists think that an athlete’s body make‑up—like muscle mass or fat—might influence the reading. In many cases, the real source of wrist discomfort is the stress injury to the growth plate itself, not just the final bone shape.
When the variance starts causing noticeable pain, doctors may consider an operation to correct the alignment. However, the best way to decide on surgery is still under debate. Future research should capture the wrist in motion using advanced imaging, include a wide mix of sports that put weight on the arms like gymnastics, cheer, diving, and wrestling, and follow athletes over time with repeated X‑rays. This will help clarify how the condition develops and whether it truly leads to long‑term problems.
